Request For CFDB Users Account

Existing CFDB Users should use this form to request an account on this website
All fields are required

This information will NEVER be passed to anyone else
We will only use this information to help us support the CFDB

Forename:		First name of the person requesting the account
Surname		Surname/Family name of the person requesting the account
Email Address		Email address of the person requesting the account This will be the User Name for logging into the account
Phone Number		Phone number of the person requesting the account A mobile/cell phone number is preferred
Password		This password will be used for logging in to the account
Membership Number		Membership number of the person requesting the account This is the Membeship number within your organisation as used in the CFDB
Territory Number		The territory number is shown in the Application Defaults of your CFDB Menu options Tools > Settings > Application Defaults
Account Type	Event Organiser Door Crew	Select the Account type There can only be one Event Organiser account for each Territory but as many Door Crew accounts as required are available
